## 3. Week 1 terms (tick one)

- [ ] Free Week. 40 hours, no fee, branch or fork only, no production access. Clause 4 and clause 8.1 to 8.3 of the Engagement Agreement apply.
- [ ] Paid first week. Up to 40 hours at the hourly rate in section 4, invoiced in advance when the SOW for that Week is accepted (Agreement 6.1). Clause 5 applies from day 1.

## 7. Terms that apply

This Order Form is governed by the Axdale Engagement Agreement version 1.0 at https://www.axdale.com/documents/ (the Agreement). This Order Form is the Schedule and the first Statement of Work under the Agreement. If this Order Form and the Agreement conflict, the Agreement applies, except for the fields filled in above. The Security and Access Policy, the Mutual NDA (if signed) and the Data Processing Agreement (if signed) also apply.

## 8. Acceptance

The Client accepts this Order Form and the Agreement when the Decision Maker does one of these 2 things.

1. Replies by email from a Client address with the sentence "I accept Order Form AX-OF-____ for ____ (company name)".
2. Signs this Order Form electronically.

Michael accepts by countersigning and sending the PDF copy to the Decision Maker. The date of the later of the 2 acceptances is the signature date under the Agreement. Where the governing law needs a special written form for a transfer of copyright, the parties sign a one-page rights transfer in that form when rights pass (O-05 Handover and Offboarding).
